Iliac side branch devices (ISBs) are “gamechangers in infrarenal therapy of abdominal aortic aneurysms (AAAs),” Mario Lescan (Tübingen, Germany) tells Vascular News. Lescan states that ISBs “allow for the preservation of the hypogastric artery […] which is very important in complex aortic procedures”.
Lescan discusses the anatomical indications for these devices and compares ISB plus endovascular aneurysm repair (EVAR) to traditional procedures such as the bell-bottom and sandwich techniques. He specifically highlights the E-liac and E-tegra devices (Artivion) which he says have “deployment systems that are very flexible which allows for excellent durability, even through tortuous iliac vessels”.
For the implantation of these devices “some basic endovascular skills are necessary” notes Lescan. “However, due to the simplicity of the system, the implantation procedure is straightforward,” he adds, before concluding that “I would recommend starting with E-tegra, EVAR or an anatomically less challenging isolated iliac side branch procedure in order to learn the procedural steps and how to deploy the system.”
